We tailor our service for young people and adults we connect you with the best Personal Assistant for your life.
Together, we will create a support plan enabling you to lead an active and fulfilled life whether at home, in education or at work. With a Personal Assistant you will be able to seize life’s opportunities, make a wider range of choices and break down the barriers to living life to the full.
Our staff are fully trained and competent to meet your complex care needs. We support people with physical disabilities, learning disabilities, chronic or terminal illness and people managing unexpected care needs, such as acquired brain injuries.

Home 2 Home with Care are passionate about supporting you to continue living as independently as possible, that’s why we provide a range of care services – Residential, Respite, Day opportunities and Community Out Reach – that can be entirely tailored to your unique needs, including companionship and full time live –in care. We understand everybody’s needs are different, so the service we provide will be tailored to your specific requirement to allow our service users to lead more fulfilling lives, whatever their health and social care needs are. We are committed to supporting people safely to retain their skills and independence as far as possible, whilst enjoying well-being and a good quality of life.
Respite and short Breaks
Having a break or a change of scene can make a real difference, not only to the lives of the people we support but to their families and carers.
Our services are tailored to a family's specific needs - enabling the family to take a break, whilst supporting the person to enjoy a positive experience. We ensure individuals can spend their budget allocation in the way that they want to.
Supported Living
Supported living is all about enabling people to make choices and be in control of their lives and supporting them in keeping them safe from harm. We encourage and empower people to achieve their dreams and goals through high quality, bespoke support.
We specialise in providing supported living for people with diverse and complex needs including individuals with a learning and/or physical disability, mental health needs, acquired brain injuries and people with autism.
Partnership
We work in partnership with local communities and agencies to deliver excellent care and help people become active members of their local community. We keep everyone, from family and case managers to the people we support, fully informed of progress and clear on the choices available. We can hold meetings with different parts of your multi disciplinary team ranging from consultants,, physios , dietitians.
Ensuring all your care needs are met by our fully trained and competent staff. We feel that individuals can over come personal barriers when the multi disciplinary team come together to share input and support.
